Participant Declaration Form

As part of the final steps in their learning journey, all participants are required to complete a Participant Declaration Form upon finishing the course at Skills Shop. This document serves as both the course registration form and a confirmation that all responses provided throughout the training were their own.

Skills Shop Ltd uses Get Formly, which allows participants to complete forms electronically. Alternatively, other forms can be used, provided they include a wet signature and track the IP address of the completed form to ensure compliance.

The participant declaration form should capture the following information:

  • Participant Name: legal name as declared on a formal document such as birth certificate
  • Course Code
  • Course Name
  • Course Completion Date
  • Reasonable Adjustments: were any adjustments made to allow the participant to complete the course?
  • Course evaluation: questions to evaluate satisfaction of the participant and trainer competence
  • Marketing Opt-in: Ask if participant details can be added to a mailing list
  • Other comments
  • Declaration – wet signature of the participant (can be signed or typed).
